Job summary
To
liaise with patients, practice staff, GPs, care home staff and community
pharmacists in all aspects of medication reviews and prescribing queries to
improve the pharmaceutical care of patients.
To support medication review and conduct medicines
reconciliation.
To identify patients suitable for medication changes
in line with the practice prescribing teamwork plan to support QOF, prescribing
targets and PCN + ICB Medicines management team projects and subsequently make
those changes with the agreement of the GP or pharmacist, e.g: dose
optimisation, formulary switching.
To assess prescribing data using practice clinical
computer systems and patient records, highlight any recommendations and
implement changes if required, to ensure patients medication records are up to
date
Under the direction of the lead Pharmacists/GPs,
assist with medication changes from hospital letters and medicines
reconciliation for new patients. To highlight any medication errors/queries and
report any concerns to the PCN Pharmacist/GPs to support local clinical
governance structures eg datix reporting.
To implement initiatives to reduce waste, improve
medicines compliance and ensure best practice is adopted in accordance with
local guidelines, legislation, safety updates e.g NICE, MHRA
To assist the lead Pharmacists with any searches,
data collection and letter production required for prescribing changes or drug
alerts.
To facilitate High Risk Drug Monitoring
Main duties of the job
To promote high quality, safe and cost-effective prescribing as
informed by evidence-based practice and national and local guidance etc. To ensure prescribing conforms to relevant
legislation.
To work collaboratively with the Brownlow Healths Prescribing
Team, the wider clinical team including GPs and nurses and support the
Brownlow Health Frailty team.
To complete structured medication reviews for polypharmacy
patients.
To work with the in-house Quality Team to support achievement of
the Impact and Investment Targets (IIF), Direct Enhanced Services Contract
(DES) and Quality and Outcomes Framework (QoF)
To respond in a timely manner to medicines safety advice e.g.,
from the Medicines and Healthcare products Related Agency (MHRA), and
disseminate to the wider team as needed
To partake in the CPPE Primary Care Pathway for Pharmacists
(ideally already completed)
To work
as part of the multidisciplinary team (MDT), to provide advice and support on
all prescribing issues. When clinically appropriate, to implement and action
changes to the patients medical prescribing record as directed by primary,
secondary or community care, working at all times within professional limits.
To
reduce the risks of medication prescribing across Brownlow Healths population.To
influence the prescribing behaviour of all members of the Practice MDT
including GPs and non- medical prescribers
