Job summary

The number of patients in Liverpool City Region living with Frailty and Dementia is predicted to continue to grow into 2040. In addressing this, Brownlow Health, supported by Central Liverpool Primary Care Network are introducing their own Frailty Service to achieve better health outcomes and quality of Life for those who are at higher risk of deteriorating health, falls, early mortality and hospital admission

Brownlow Healths Frailty Service will be a multidisciplinary team providing proactive care primarily for patients living with moderate and severe frailty. This will include patients who are housebound or would have difficulty accessing services and will therefore require home visits. Patients may be living in their own home or a care home and will typically be living with multiple co-morbidities and polypharmacy or may be approaching end of life. If you would like the opportunity to be part of this new and exciting service, we are currently recruiting a Nurse Associate who has experience working with older adults and frailty as well as long term conditions

You will receive a 6 month induction and mentorship package and salary and annual leave in line with Agenda for Change. As this is predominantly a home visiting service you will require a full driving licence and access to a vehicle for which you will receive a generous mileage allowance

Main duties of the job

  • Lone visits to moderate and severely frail housebound patients/Care Homes
  • Frailty screening and identification through onsite frailty clinics and telephone consultations
  • Post Hospital discharge monitoring/follow up as directed by senior clinician.
  • Monitoring of long-term conditions/annual review - QOF related, with complex patients referred to either the Physician Associate or Advanced Nurse Practitioner.
  • Administration of Flu, Pneumonia, Shingles, B12 vaccines under PSD
  • Check medication compliance, demonstrate how to use new blister packs and educate/support where necessary with inhaler technique.
  • Accurately perform and record all clinical observations including: blood pressure both digitally and manually, temperature, respiration rate, pulse, SPO2, blood glucose monitoring, weight/BMI and urinalysis. Have an understanding of their clinical values and know when to escalate.
  • Perform annual Diabetes foot check for housebound if needed.
  • Perform a range of clinical and non- clinical referrals.
