Part time role for up to 6 hours per week with a negotiable salary

MAIN DUTIES 1. Undertake assessments, in the GP surgery and or at home if housebound, and create Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being through face to face and virtual contacts, including within the individuals own homes where appropriate. 2. Provide service users with continuity and a coordinated experience of care, remaining point of contact throughout the individuals social prescription. 3. Establish and maintain effective liaison and build relationships with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary. 4. Work in partnership with voluntary and community organisations to contribute to a comprehensive and up to date database of local resources to support, promote and improve the Social Prescribing Role. 4 5. Support and develop GPs and primary care health teams knowledge on how to identify patients suitable for social prescribing service referral.

In addtion to the above;
6. Report to the Senior Management Team 7. Keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required. 8. Ensure you have an understanding (appropriate to your role) of and comply with safeguarding procedures to protect the welfare of children and vulnerable adults. 9. Comply with the practices Health and Safety Policy, Data Protection Policy and to protect the health, safety and welfare of yourself and others. 10. Work flexibly as required by the Practice.
To accept , review and act upon referrals received from GPs and primary care staff. Referrals can be generated through proactive searches or through reactive referrals following patients interaction with the practice. A holistic assessment will be carried out where required and individuals directed and sign-posted to the most appropriate provider of the services they need.
