Job summary
The University of Nottingham Health Service require a highly motivated and experienced Practice Nurse, to join our dedicated multi-disciplinary healthcare team. Previous work in general practice preferred as is experience in managing long term conditions.
The position is for 21 hours per week; to work
Mondays - Flexible depending on Candidate
Wednesdays -10am- 6pm
Thursday 7.30 3.30
And approximately 4 Saturday mornings per year.

Main duties of the job
To deliver high-quality, evidence-based nursing care to patients within the University of Nottingham Health Centre (Cripps Health Centre)
To work as part of a multidisciplinary team. The role focuses on health promotion, disease prevention, and the management of long-term conditions in line with NHS policies and guidelines.

Provide travel health advice and vaccinations (where trained)
Health Promotion & Patient Education
Promote health and wellbeing through lifestyle advice (smoking cessation, diet, exercise)
Support patients in self-management of chronic conditions
Contribute to public health initiatives and screening programmes

Participate in the administrative and professional responsibilities of the Practice team
Ensure that accurate notes of all consultations and treatments are recorded in the patients notes
Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice
Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
