We are seeking a dedicated Care Home Practice Link Nurse to join our team and serve as a vital connection between our aligned care homes and the Practice. This exciting and rewarding role will enable you to:

As part of this role, you will link with the Practice Nurse and the wider Practice Team, ensuring continuity of care across our Practice and driving forward best practices in elderly care.
Main duties of the job
In the role of Care Home Practice Link Nurse, you will be responsible for providing high-quality nursing care to residents in care homes while acting as a vital link between the care homes and the general practice team. You will be involved in assessing, planning, and coordinating the healthcare needs of residents, providing clinical advice and education to care home staff, and supporting the integration of care within the primary care network.

- Act as the primary point of contact for designated care homes, building effective working relationships with care home staff, residents, and families.
- Conduct comprehensive nursing assessments for care home residents, including long-term condition reviews, medication reviews, and frailty assessments.
- Coordinate multidisciplinary team (MDT) reviews, ensuring all stakeholders are involved in care planning.
- Support early identification and management of deterioration in residents health
- Provide advice on evidence-based clinical interventions, including end-of-life care and palliative care management.
CLINICAL NURSE DUTIES
- Undertake clinical duties such as wound care, immunisations, chronic disease management, and phlebotomy where required.
- Deliver proactive care, including health promotion and preventative healthcare for residents.
- Liase with and refer to the appropriate services for ongoing patient care management.

- Experience in primary care, community nursing, or care home settings. Evidence of post-registration training, e.g., chronic disease management or advanced clinical assessment.
- Infection control awareness and ability to maintain working standards. Understanding of human needs - physical, emotion social.
- Ability to recognise and manage risk.
- Working with confidential information and an understanding of service user confidentiality.
- The needs of vulnerable adults, safeguarding and the associated legislative framework.
- An understanding and awareness of the Enhanced Health in Care Homes Framework Working in a multi-disciplinary setting.
- Continuous CPD and identifying opportunities for self-development. Able to work autonomously.
- Ability to work to high standards of accuracy.
- Ability to problem solve.
Desirable
- Qualification in long-term condition management (e.g., Diabetes, Insulin Initiation, COPD, Asthma. Heart Failure,).
- Experience in end-of-life or palliative care.
- Training in frailty assessments.
- Willing to undertake the relevant training as required. Independent/supplementary nurse prescribing qualification.
- Clinical supervision training and experience.
- Specialist knowledge of end-of-life care planning.
- Knowledge and experience of using clinical software e.g. SystmOne. Knowledge of Excel and Word.
