The Living Well Partnership was established in 2017 following the merger of four GP surgeries and three smaller branch sites. Today, we comprise eight surgeries with ten partners, united by a shared vision: to deliver more seamless, efficient, and patient-centred medical care by pooling our resources rather than operating as separate practices.

Since forming the Partnership, our patients have benefited from faster access to a broader range of services and improved behind-the-scenes efficiencies. As of March 2023, we are proud to be launching our Continuity Project initiative designed to further enhance the consistency and quality of care we provide.

We care for more than 48,000 adults and children across the eastern Southampton region. With eight well-located sites, we ensure that everyone from inner-city residents to those in rural communities has convenient access to their local GP.

Our innovative continuity model features 12 dedicated GP teams, each responsible for a personal patient list of around 3,500 people. These teams are supported by a committed Care Coordinator and Care Coordinator Assistant, as well as the shared clinical expertise within our Partnership. This includes Senior Non-Principal GPs, Non-Principal GPs (including retainers), Advanced Nurse Practitioners, Practice Nurses, Mental Health Nurses, Specialist Nurses in Respiratory and Womens Health, First Contact Practitioners for Physiotherapy, Nurse Associates, Healthcare Assistants and in-house Pharmacy team of Clinical Pharmacists and Pharmacy Technicians.

Together, this ensures every patient receives truly holistic, coordinated care.
