Job summary
We are looking to recruit a qualified Advanced Nurse Prescriber to join our
motivated, friendly and supportive GP Practice in Shere, Surrey. The successful
candidate will be confident in independently managing acute cases of minor
illnesses arising from the clinical triage system. Seeing and reviewing
patients with long term conditions. Supporting the triage and care navigation
of patients as well as providing care to patients face to face, over the
telephone and sometimes in their own homes.
Main duties of the job
The
Advanced Nurse Practitioner (ANP) will work autonomously within the GP Practice
team to provide advanced clinical care, including assessment, diagnosis,
treatment, and management of patients presenting with undifferentiated and
undiagnosed conditions. The ANP will also support chronic disease management,
health promotion, and the delivery of evidence-based care.

Clinical Care:
- Provide care to patients
with acute, chronic, and complex conditions.
- Undertake clinical
assessments, initiate investigations, diagnose, and formulate management
plans.
- Prescribe medications within
the scope of professional practice.
- Refer patients to
appropriate secondary or specialist services when needed.
- Perform clinical procedures
within the scope of practice (e.g., wound care, joint injections, cervical
smears, etc.).
- Lead chronic disease clinics
(e.g., asthma, diabetes, hypertension).
Autonomous Practice:
- Manage a caseload of
patients independently, with access to GP support as needed.
- Recognise and act upon red
flag symptoms and signs of serious pathology.
- Maintain accurate, timely,
and legible clinical records.
Collaborative Working:
- Work closely with GPs,
nurses, pharmacists, and other multidisciplinary team members.
- Participate in practice
meetings, clinical audits, and quality improvement initiatives.
- Support triage and patient
flow during high-demand periods.
Development:
- Contribute to clinical
governance, risk management, and safeguarding procedures.
- Participate in ongoing
professional development and appraisal processes.
Health Promotion and Education:
- Provide education and advice
on lifestyle, prevention, and self-care.
- Participate in patient
engagement initiatives and local health campaigns.

- Desirable Skills:
- Experience using EMIS Web or other GP clinical systems.
- Skills in minor illness, minor injury, or womens health.
- Ability to work well under pressure and adapt to changing demands.
