Job summary
CYGNET Primary Care Network (PCN) is looking to recruit an
innovative and enthusiastic Paramedic / Advanced Clinical Practitioner to work in
primary care.
The successful candidate will deliver new and innovative services to
meet the needs of our patients. You will work as part of a multi-disciplinary
team as a key first point of contact practitioner for acute presentations. The
role will deliver clinical care to patients, supporting clinicians, practice, and primary care staff within CYGNET PCN. We are adopting a holistic approach to
assessment, diagnosis, and treatment.
Pay is negotiable depending on experience. The position
is full-time and benefits from NHS
Pension scheme membership.

Main duties of the job
Key areas of work:
Clinically examine and assess patient needs from a physiological and
psychological perspective, and plan clinical care accordingly
Take a comprehensive patient history, conduct physical examinations, and use expert knowledge and clinical judgment to identify the potential
diagnosis
Undertake the collection of pathological specimens including intravenous
blood samples, swabs etc. Perform investigatory procedures requested by the
clinicians. Refer patients for investigations and/ or the appropriate specialist
where appropriate
Use practical experience to plan and provide skilled and competent care
to meet patients' health and social care needs, involving other members of the
wider health care team as appropriate.
Liaise and seek advice from appropriate healthcare professionals
Access to a vehicle and a full driving licence is essential for this
post.

network comprises of a range of roles from Clinical Director to Care
Co-ordinator.
PCN's build on existing primary care services and enable greater
provision of proactive, personalised, coordinated and more integrated health
and social care for people close to home. Our PCN are proactively providing
care and services for the people and communities we serve.
New
roles are being introduced to the network as we are expanding into different
areas of healthcare.
The
network provides a single point of access for the nursing, residential and
learning disability homes in the area.
