Care Coordinator Prescription Clerk

Job summary

The care coordinator will be part of the Severnvale Primary Care Network (PCN). The role will involve coordinating with the work of healthcare professionals and non-clinical staff involved in the care of patients. The post holder will contribute to tackling inequalities in health and social care particularly regarding individuals with long-term conditions including supporting the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation plus reducing medicines waste and maximising patient outcomes. The Care Coordinator Prescription Clerk will work to support patients medication management in care homes plus patients who are assessed to require medication compliance aids.

We are looking to recruit 1 member of staff to work the following hours

Monday 10am - 5.30pm

Tuesday 10am - 3pm or 11am-4pm

Wednesday 10am-3pm or 11am -4pm

Friday 10am - 5.30pm

Full training will be provided.

Main duties of the job

  • To process repeat prescriptions on a daily basis ensuring compliance with the Prescription Hub protocols and standard operating procedures. All in an acceptable timeframe.
  • To deal with prescription queries from patients, carers, GP practice staff, community pharmacy staff, appliance suppliers, care home staff and healthcare professionals.
  • Perform good house-keeping of repeat medication, e.g. medication reconciliation, medication synchronisation, removal of old medication as guided by the pharmacy technicians/pharmacists.
  • Effectively liaise with external services, i.e. Community Pharmacies
  • Providing patients with high quality, easy to understand information to assist them in making choices about their care and assisting with queries around medication with support of the PCN Pharmacy Team.
  • Proactively identifying patients through admissions and discharge data
  • Creating and maintaining accurate and relevant records, including in patient medical records and communicate progress or issues with managerial or clinical staff as appropriate
  • To provide excellent customer care, demonstrating empathy, patience, and a holistic approach to patient care, with commitment to follow-through care plans and build effective working relationships
  • Coordinating the monitoring of patients long-term conditions where appropriate

Severnvale PCN (Primary Care Network) comprises five GP practices in South Gloucestershire delivering services to a population of circa 33,500 patients which includes 8 care homes. We are an enthusiastic, dynamic, and friendly PCN who constantly strive to improve patient pathways and health care outcomes.

The PCN team includes a Clinical Director, a PCN Manager, a Community Frailty Practitioner, a Care Coordinator, 4 Clinical Pharmacists, a Pharmacy Technician, 7 Care Coordinator Prescription Clerks, 2 dedicated Social Prescribing Link Workers and First Contact Physiotherapists

Primary Duties & Areas of Responsibility

Creating and maintaining accurate and relevant records, including in patient medical records and communicate progress or issues with managerial or clinical staff as appropriate

Working closely with GP practices and PCN healthcare staff to identify and work with a cohort of people to support their personalised care requirements

To provide excellent customer care, demonstrating empathy, patience, and a holistic approach to patient care, with commitment to follow-through care plans and build effective working relationships

Coordinating the monitoring of patients long-term conditions where appropriate

Work with patients to understand their level of knowledge, skills and confidence when engaging with their health, wellbeing and prescribing needs

To support in the delivery of Quality and Outcomes Framework, enhanced services and other service requirements on behalf of the PCN

To contribute to public health campaigns (e.g. flu and Covid vaccination clinics)

To understand the common need and safeguarding issues of vulnerable patient groups, including the elderly; housebound and those with long term conditions including physical and mental disabilities

To undertake all mandatory training and induction programmes

To attend a formal appraisal with your manager at least every 12 months. Once performance/training objectives have been set, progress will be reviewed on a regular basis so that new objectives can be agreed.

To work weekends as and when required
