An exciting opportunity has arisen for an Advanced Nurse Practitioner to join our team.
We are a busy, high-achieving, well-organised organisation running two Central London Practices with 15000+ patients. We are open 7 days a week and provide different services that cater to our patients' needs.We are easily accessible with both locations based in Central London.
We are looking for a highly motivated and suitably experienced Nurse/Advanced Nurse Practitioner to join our Clinical Team. Working collaboratively with the Practice multi-disciplinary team, you will play an integral part of the team assessing patients using different forms of consultations, i.e. face to face, telephone, videoand online, and formulating a plan of care. You will be required to maintain and develop further your theoretical knowledge and physical examination skills.
Candidates are invited to apply who have relevant autonomous clinical experience and competence in caring for patients in different settings, e.g., acute medicine, emergency and out-of- hours service. Evidence of advanced assessment skills and completion or in process of completion of your non-medical prescribing is desirable.
A commitment to the advancement of nursing within primary care, together with excellent communication skills and ability to work well autonomously and within a team are essential for this role.
An interest in Mental Health, Respiratory diseases, Older Persons or Children/Young People is highly preferred but not essential.
Main duties of the job
1. Work as part of the multi-disciplinary team to deliver safe and high quality care.
2. Carry out clinical nursing practice at an advanced level using role-appropriate clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
3. Triage and treat patients, assess their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
4. Prescribing and reviewing medications appropriate to the scope of the role.
5. Diagnose and manage acute and chronic conditions.
6. Request tests and investigations rationally, and interpret results
7. Assist and support the practice' performance related clinical remits e.g. QOF, Partnership In Practice, and Prescribing to name a few.
8. Work with the practice administrative team and help streamline service delivery
9. Collaborate with other professionals in the community to assist patients who need support.
