Job summary

To be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention as well as supporting the management team in the reviewing of clinical policy and procedure.

Main duties of the job

The practice nurse will work as part of the primary healthcare team providing chronic disease management and primary medical services to patients. The practice nurse will take a lead role in coordinating and organising the practice recall system in line with QOF requirements.

Patient Services

  • New Patient Medicals
  • Travel Vaccinations
  • Cytology
  • Asthma / COPD / Respiratory Management
  • Hypertension, Diabetes, Coronary Heart Disease and Contraception Management
  • Venepuncture
  • Routine Immunisations and Childhood Vaccinations
  • Request and process results of pathology tests for example urine culture / swabs and bloods
  • Following agreed clinical protocols with referral to senior nurses / GP as appropriate
  • Active Participation in administrative systems
  • Attend and participate in practice meetings when required
  • On occasion there may be a requirement to undertake a home visit
  • Any other delegated duties appropriate to the post
  • Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies

About us

We are a single site Practice with over 11 800 patients and a clinical team consisting of 4 Partner GPs, 2 Salaried GPs 1 Nurse Practitioner, 1 Trainee Nurse Practitioner, 3 Practice Nurses and 3 Healthcare Assistants as well as various PCN roles.

Qualifications

Essential

  • Registered Nurse with the Nursing and Midwifery Council

Desirable

  • Postgraduate diploma or degree (Primary Care)

Experience

  • Experience of chronic disease management
  • Experience of working within a multi-disciplinary team

  • Experience of working in a Primary Care environment
  • Experience of working as a Practice / Community Nurse
