Job summary

Due to upcoming retirement, St Luke's Surgery, Bloxwich are recruiting for an Advanced Nurse Practitioner.

We are a dedicated, friendly team that aims to deliver the best possible care to our registered patients.

Hours are negotiable and salary will depend on previous experience.

NHS Pension available.

Main duties of the job

The successful candidate will act as an autonomous practitioner working independently and in conjunction with other health care professionals.

They will assess, diagnose and treat patients by holding minor ailment clinics. Practice Nurse duties and chronic disease management will also be involved.

All candidates should have an Independent Prescriber Qualification and be registered with the NMC. Previous experience of working in a GP Practice would be beneficial.

About us

St Luke's Surgery has around 5400 registered patients. We are a well established team with approx. 21 staff. We are a training practice and currently have 3 trainee GPs working with us.

As a practice we pride ourselves in delivering the highest standards of care for our patients as well as providing a happy, well supported environment for our staff.

We strive for a high QOF performance and GOOD CQC reports.

Job description

Job responsibilities

The successful candidate will be an experienced nurse who will act within their professional boundaries to provide the best possible care to presenting patients. This will include taking their clinical history, completing clinical examinations, diagnosing and prescribing appropriate treatment.

They will demonstrate safe, clinical decision making at all times by working within Practice guidelines, policies, protocols and procedures reporting outcomes to appropriate clinical staff so standard of care can be monitored and improved where necessary.

Delivering chronic disease management to avoid hospital admissions by creating a personalised care plan involving the patient.

Working with multidisciplinary teams within the practice and across the wider NHS to promote integrated pathways of care.

Recording accurate consultation data in patients records in accordance with the latest NMC guidance and other pertinent standards.

The post holder is required to respect patient confidentiality at all times and not divulge patient information unless sanctioned by the requirements of the role.

Delivering health screening, contraception, dietary advice, blood tests, urinalysis, ECGs and blood pressure checks (Nursing Duties list is non-exhaustive).

Contributing to achieve high targets.

Supporting other Nurses in developing and maintaining clinical skills.

Maintaining effective communication with patients, Carer's, relatives and staff.
