We are looking to recruit an experienced Practice Nurse to join our dynamic and forward thinking team. We are based in Litherland and offer a modern and bright place of work. We are a training practice and actively encourage learning and development across our team.
The successful candidate must be able to demonstrate good clinical skills in respect of chronic disease management, vaccination and immunisation programmes, shared care monitoring and cervical cytology.
Main duties of the job
The successful candidate will be required to undertake consultations to deliver chronic disease annual reviews deliver vaccine and immunisation programmes, cervical cytology and some required phlebotomy. They would also be expected to participate in clinical audit, health & safety and infection control assessments.

- Maintain high standards of care within NMC professional Code of Conduct
- Maintain own PREPP requirement
- Participate in Clinical Supervision by arrangement with Practice
- Maintain own clinical/professional expertise by participating in clinical activities
- Develop philosophy of care using a research based approach
- Maintain effective liaison with other members of PHCT and other agencies
Clinical Duties:
- Responsible for management of treatment and continuing care for patients on an open access basis
- Overall responsibility for nursing management of groups of patients, for example, diabetes, asthma, hypertension, including responsibility for developing, implementing and evaluating programmes of care as per practice protocols
- Provide advice and general health care/minor ailments with referral to GP as appropriate
- Provide health promotion advice where appropriate with referral to GP as appropriate
- Provide and maintain a treatment room service to the practice population
- Participate in immunisations of children/adults, for example tetanus boosters, holiday vaccinations, and influenza/pneumococcus
- Provide cervical smear taking in the practice
- Provide family planning within the practice
- Provide health assessment services
- Assist in minor surgery
- Control administration to own case list
- Assist in the teaching of other nurses, GP registrars & HCAs etc.
- Provide nurse led triage as agreed by current practice protocol
- Will be involved in selecting, carrying out and interpreting specific tests
Administration:
- Ensure that all records are up to date and fully documented including putting data on to a practice computer as necessary
- Complete practice administrative tasks as necessary
- Assist in the formulation of practice protocols
- Involvement in practice audit and or research
- Report and record all complaints and participate in any investigations as necessary
- Attend practice protected learning events and educational meetings/events
- Attend South Sefton PCN and Locality meetings where necessary
Management:
- Establish and maintain good communication and working arrangements with all other services and all members of the locality teams to ensure effective smooth deliver of care
- Exercise leadership and aim to achieve high morale within the nursing team by efficient management skills, good staff relationships and good clear lines of communication
- Give support to junior staff
- Co-ordinate the educational activities within the team
- Ensure staff awareness of and conformity with nursing policies and procedures
- Identify care needs within the practice population and organising work to best meet the needs identified
- Provide nursing input to any decision making process affecting patient care

- To clean and maintain all equipment and organise sterile supply
- Maintain stocks and order as necessary
- Maintain cold chain
