Job summary

Are you interested in being a part of a small and friendly team in the South Cotswolds Primary Care Network? We are looking for a full-time band 4 community based Wellbeing Coordinator to join the South Cotswold Frailty Service working with people in their own homes. The post holder will hold a caseload and work with people with frailty to understand what is important to them, to improve quality of life, functional capability and independence. Personalising the care of people with frailty helps to avoid unnecessary hospital admissions, maximises independence, and proactively supports people to manage the symptoms of their long term conditions.

This is a permanent role, and the hours of work will be between 8.00am and 6.00pm (Monday-Friday) although in the future expansion of hours may be considered to include 7 day working.

Main duties of the job

The post holder will be linked to GP practices identified within the South Cotswolds Primary Care Network. They will work closely with colleagues across health, social care and third sector organisations to support holistic and person centred care at home. The role involves:

Undertaking screening assessments to determine the referral into the service.

Providing a timely assessment to those patients who have symptoms of frailty and who have been identified as benefitting from the service.

Supporting those living with frailty to achieve prolonged quality of life, improved functional capability and independence, and help keep patients living with frailty from slipping into the more severe frail (2%) cohort of patients

Supporting patients with an opportunity to attend and participate in social activities within the locality or, in the current environment to minimise the impact of social isolation and the new world we live in.

Supporting patients to identify and achieve their goals and improve quality of life.

Acting as a single point of contact for patients on caseload.

Using a case management approach, the team promotes independence and self-care. The post holder will work in collaboration with the GP Practice, Community Matron and external providers to create a personalised and coordinated care plan for identified people with frailty. The post holder will be responsible for developing excellent working knowledge of support available from partners in health, social care and the voluntary sector to encourage and guide this group of individuals to access these services where appropriate.

With the support of the Community Matron, the Wellbeing Coordinator will be responsible for supporting people with frailty in the South Cotswolds by maximising the persons potential for independence and working with health, social care and third sector organisations. The Wellbeing Coordinator will take a case management / advisory approach for the identified cohort of patients with frailty and will act as a single point of contact for the people whose cases they are managing. They will develop relationships and an understanding of the people on the caseload that will enable them to spot a decline in health or warning signs sooner and more readily.
