Job summary

A fantastic opportunity has come up for a registered Nurse to join our multi-disciplinary team. We are looking for a candidate who is enthusiastic, forward thinking, dedicated and can demonstrate the ability to give a friendly, caring, and professional nursing service to our patients.

Previous primary care experience would be an advantage, but not essential as for the right candidate, full training will be given.

Main duties of the job

Routine Immunisations/Childhood Immunisations/Flu/Covid vaccination clinics

Develop, implement and embed health promotion and wellbeing.

Support the clinical team in the development of individual treatment plans for chronic disease patients.

Identify and manage and support patients at risk of developing long term conditions, preventing adverse effects on patient health.

Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and National Service Framework guidelines.

Provide wound care/doppler.

Provide Travel medicine advice.

Provide Long Term Condition clinics, delivering patient care as necessary.

Maintain accurate clinical records.

Cytology

Chaperoning and assisting patients where appropriate who are being examined by another clinician.

Following agreed clinical protocols with referral to Senior Nurses or GPs

Phlebotomy and ECGs

We are a friendly training practice that uses SystmOne, with approx. 8000 patients currently registered with us. As a practice we form part of the Heartbeat Alliance along with the Enhanced Access in Hambleton and Richmondshire.

Job description

Job responsibilities

Undertakes procedures to include, venepuncture, glucose tolerance tests, BP management, urinalysis, peak flow, spirometry, new patient health checks, cytology, complex wound management, ECGs.

Administers adult and child immunisations and vaccinations in accordance with national and local programmes. Is competent in anaphylaxis and resuscitation techniques. Ensures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.

Recognises and understands the importance of observing, documenting and communicating the signs and symptoms of child and adult abuse.

Works within all relevant practice policies and procedural guidelines e.g., infection control, chaperoning and patient group directions.

Takes a lead in specialist clinical areas within own competencies (e.g., CHD, family planning etc) and undertakes the establishment and running of disease management clinics providing specialist nursing care for the patients and communities that the practice serves.

Initiates the process of diagnosis with patients suspected to have a chronic disease e.g., diabetes, COPD, asthma, CHD referring to other clinicians as appropriate.

Manages programmes of care for patients with chronic diseases by planning, providing and evaluating care under agreed local/national guidelines, protocols and patient group directions.

Maintains disease registers.

Audits outcomes of care against standards and initiates changes as necessary.

Undertakes annual review of patients understanding and ability to self-manage.

Refers to other members of the extended primary health care team eg chiropodist, dietician, specialist nurses.
