An old established town house, GP surgery who are looking at employing a new practice nurse to the team, which still give the "family doctor" feel with face to face appointments.
We want to continue improving patient access, patient experience and health outcomes and look forward to another team member joining the existing friendly, supportive nursing team to do this.
The patient feed back is very positive of all of the team members for the support and services provided
Main duties of the job
Responsible for the clinical areas of the primary care nursing team within a GP practice. Delivering general treatment room services including cervical screening, child immunisations, health promotion, chronic disease management, wound management, travel services, ear irrigation/micro suction
Ensuring that the nursing services are delivered effectively to the patient population and working with the practice multidisciplinary team in accordance with clinical based evidence, NICE and the NSF
Supporting patients through their diagnosis and signposting to the relevant support agents.

Ran by three Partners, 2 are Male and 1 Female they are supported by three salaried GP's, 2 are Female and 1 Male.
Currently the nursing team is made up of a very experienced ANP who has served many years within the practice and developed skills from being a Practice Nurse to becoming an ANP; an experienced Practice Nurse who has also served many years with the practice having started her nursing career as a midwife and then 2 Health Care Assistants.
The practice is managed by a full-time practice manager and there are a number of part-time staff who cover reception, administration and secretarial roles.
Attached to the surgery is an MSK consultant and extended access GP and HCA.
The surgery opens at 7.30 a.m. for surgeries starting at 7.40 a.m.; on a Monday one of the Partners offers extended hours until 7.30 p.m. but otherwise the surgery is closed at 6.30 p.m. We are open Monday to Friday unless we are carrying out immunisation programmes
We are keen to have staff who may like to extend their experiences and qualifications by learning new skills; we are also involved in our local PCN and their development of training practices.

- To support the Infection Prevention Control (IPC)
lead and to work to the IPC Policy supporting the implementation of audits,
training, support and evidence of compliance to the practice manager
- To assess the needs of patients ensuring the provision of healthcare is
appropriate, incorporating evidence-based practice
- To develop, implement and embed health promotion and wellbeing programmes
- To implement and evaluate individual treatment plans for chronic disease
patients that promote health and wellbeing
- To identify, manage and support patients at risk of developing long-term
conditions, preventing adverse effects to patients health
- As required, to provide routine nursing care to patients in accordance with
clinical based evidence, NICE and the NSF
- As required, to diagnose and manage acute and chronic conditions
- To prescribe medication for patients in accordance with PGDs and own scope of
practice
- To review patient medications to enhance compliance
- To provide guidance to patients in the use of prescribed and over-the-counter
medications regarding side effects and dosages
- To coordinate the provision of travel medicine services
- To request pathology services and process pathology results as required
- To oversee the provision of chronic disease clinics, delivering patient care
as necessary, referring patients to secondary/specialist care as required
- To maintain accurate clinical records in conjunction with extant legislation
- To ensure the use of templates by all nursing team
- To ensure the appropriate maintenance and use of chronic disease registers
- To carry out cervical screening
- To develop, implement, embed and participate in well man clinics
- To prioritise health issues and intervene appropriately
- To support the team in dealing with clinical emergencies
- To recognise, assess and refer patients presenting with mental health needs
- To implement vaccination programmes for adults and children
- To support patients in the use of their prescribed medicines or
over-the-counter medicines (within own scope of practice)
- To liaise with external services/agencies to ensure the patient is supported
appropriately(vulnerable patients etc.)
