Job summary

Well Up North Primary Care Network is looking to recruit a Pharmacist to continue to improve the quality and safety of care delivered to our patients across our 7 member practices. Pay will be dependent on experience.

This is an exciting opportunity to be part of the established primary care Pharmacy Team in North Northumberland. You can make a direct and positive impact on patient care and develop your clinical expertise and skills, all whilst helping the PCN achieve key DES requirements

Previous experience in primary care is not essential, but would be advantageous. We welcome pharmacists from all backgrounds and settings, and offer flexibility around working hours and will consider part time applicants.

Main duties of the job

The post holder will work alongside our Lead PCN Pharmacist, David Pearse, and other clinicians within the Well Up North Primary Care Network Pharmacy Team that can act across the networks patient list and member practice sites. This will be a patient-facing role, and will form part of a multi-disciplinary team to improve patient health outcomes.

Responsibilities will include chronic disease management and proactive review of specific patient groups through structured medication reviews. Medicines management and optimisation will be an expected process of all review work. Clinical audits, prescription safety management, repeat prescription management, acute prescription request management, medicines reconciliation, oversight of prescribing processes across the network, supporting and advising prescribers across the network and supervision of colleagues.

The post holder will be working as part of the Well Up North Primary Care Network (PCN), a combined list of over 62000 patients and a group of 7 practices: Alnwick Medical Group, Belford Medical Group, Gas House Lane, Wooler Surgery, Greystoke Surgery, Union Brae and Well Close Medical Group.

Access to the 18 month full funded and accredited CPPE Primary Care Pharmacy Education Pathway, with an opportunity to complete independent prescribing for non-prescribers as an addition to the pathway. Additional support, supervision, mentoring and CPD opportunities will also be provided. Clinical mentorship will be provided by a named GP lead and our Lead Pharmacist.

The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside general practice teams.

The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice. This post will also include an accredited training pathway for pharmacists (the CPPE Clinical pharmacists in General Practice education programme) to enable the post holder to practice and prescribe safely and effectively in a primary care setting.

  • Masters degree in pharmacy (MPharm) (or equivalent)

Desirable

  • Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience. Independent prescriber or working towards/intent of gaining independent prescribing qualification

Experience

Essential

  • Minimum of 2 years post-qualification experience. An appreciation of the nature of GPs and general practices. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.

Desirable

  • In depth therapeutic and clinical knowledge and understanding of the principles of evidencebased healthcare. Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
