We have an exciting opportunity for a Lead
Practice Nurse to join our friendly and forward-thinking team at Slaithwaite
Health Centre. A high-achieving, training practice located in a vibrant,
semi-rural setting.
This pivotal role is ideal for a skilled, passionate, and motivated individual
who is ready to provide leadership and support to our well-established nursing
team. The successful candidate will play a crucial role in the ongoing
development and delivery of high-quality, patient-centred care for our growing patient
population. The post is for 30 hours per week over 4 days

Main duties of the job
Clinical Leadership
- Provide effective clinical leadership to
the nursing team.
- Champion evidence-based nursing care and best practices.
- Lead on clinical governance, quality assurance, and safety initiatives.
Patient Care & Chronic Disease Management
- Oversee chronic disease clinics and
long-term condition management.
- Deliver high-quality care across treatment planning, implementation, and
review.
- Promote proactive health education and disease prevention strategies.
- Deliver own nurse-led clinics (e.g., asthma, diabetes, hypertension).
Team Management & Mentoring
- Supervise, mentor, and appraise nursing
staff and students.
- Support professional development and performance improvement.
- Manage nursing rotas to ensure effective clinic coverage and staff balance.
Policy, Compliance & Clinical Standards
- Develop and review nursing protocols and
clinical procedures. Ensure compliance with CQC regulations and NHS standards. Participate in audits, QI initiatives, and root cause analyses for incidents.
Collaboration & Communication
- Work closely with GPs, the Practice
Manager, and multidisciplinary colleagues.
- Liaise with external agencies, community services, and safeguarding teams.
- Represent nursing in practice meetings and strategic planning.

- Cervical cytology and womens health services. -

- Wound care, suture removal,
- ECGs,
- Venepuncture,
- NHS Health Checks and opportunistic health promotion
- Safeguarding children and adults;
- Mental health assessments
- Requesting/interpreting pathology results
- Referral coordination Infection prevention and control (IPC)
- Support with audit cycles and service evaluation
- Ensure proper use of SNOMED CT coding and chronic disease registers
- Uphold best practice in medication management and PGDs Encourage team learning and contribute to the practice's continuous improvement
