Job summary

We are looking for applicants who are energetic, enthusiastic and focused on delivering a high standard of care to our patients. You should be proficient in all aspects of general practice nurse duties, management of Long-Term Conditions is advantageous.

Main duties of the job

To work as part of the primary healthcare team, providing medical services to the practices patients; including treatment, preventive care, screening and patient education.

  • Demonstrate evidenced based knowledge in the main physical health problems that adult patients may present with i.e., high blood pressure, diabetes, obesity, epilepsy, respiratory issues, constipation, lack of personal hygiene, including foot care.
  • Demonstrate an ability to undertake baseline physical observations and record results correctly. In addition, be able to identify anomalies that should be referred to a first level nurse or one of the medical staff Baseline observations include:Phlebotomy,Blood pressure, Pulse, Respirations, Weight, Height,Glucose monitoring
  • Demonstrate an ability to complete ongoing monitoring templates. After undertaking additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN, in line with the competencies of the role.
  • Promote health and well-being to all patients, for example undertaking the NHS health checks.
  • care for individuals with complex conditions such as dementia, mental health conditions, and learning disabilities

About us

We are a small friendly, supportive PCN, seeking to appoint a Nursing Associate nurse/Registered to join our team. The role will be based at each practice over the week.

We care for approximately 28K patients using SystmOne, comprising of four GP Practices Grafton Road Surgery, Hobs Moat Medical Centre, Richmond Medical Centre and St Margarets Medical Practice. Our team works very well together and provides essential, considerate patient care.

Job responsibilities

Meet the specific qualification and training requirements as specified in the Nursing Midwifery Standards of proficiency by undertaking the two-year Foundation Degree delivered by a Nursing and Midwifery Council (NMC) - approved provider

is registered with the NMC and revalidation is undertaken in line with NMC requirements

Work with other members of the nursing and healthcare team to organise and provide nursing services for the practice population

Provide nursing treatments to patients in participation with General Practitioners (GPs) or independently, following agreed protocols

Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to GPs as necessary

Advise patients on general healthcare and minor ailments with referral to GPs as necessary

To engage in positive relationships with patients/carers/families and other staff based on respect and within appropriate boundaries.

To act in a professional capacity at all times.

To be able to plan and manage workload.

Key responsibilities in relation to delivering health services:

work as part of the PCNs Multidisciplinary team (MDT) to provide and monitor care, under direct or indirect supervision[1]

improve safety and quality of care at every opportunity

contribute to the delivery of integrated care

work with the PCN MDT to ensure delivery of nursing associate duties complement existing workforce

provide support and supervision to Training Nursing Associates, Healthcare Assistants, Apprentices, and those on learning assignments/placements as required

support registered nurses to enable them to be able to focus on the more complex clinical care

develop relationships across the MDT to support integration of the role across health and social care including primary care, secondary care, and mental health
