Job summary:

The practice nurse will work as part of the primary healthcare team providing Medical Services to the patients of the practice.

Job responsibilities:

Essential general treatment room duties to include:

  • Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs
  • Wound care / ulcer care/Doppler
  • Immunisations
  • Chronic diseases
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Requesting pathology tests, for example urine culture, swabs
  • Following agreed clinical protocols with referral to senior nurses or GPs as appropriate
  • Maintain general tidiness and cleanliness of nurses and treatment rooms
  • Cytology
  • ECGs
  • Asthma/COPD
  • Venepuncture
  • Routine immunisations/Childhood immunisations
  • Diabetes
  • Cardiovascular disease

New Court Surgery is located in Malvern and operates from modern purpose built facilities.

Our list size is over 10,300. Our aim is to provide high quality, accessible medical care to our patients, whilst maintaining a supportive and pleasant working environment for clinicians and staff.

We are part of Hereford and Worcestershire ICB, and the Malvern Town PCN and offer General Medical Services to our patients.

We are a training practice.

The practice has 4 Partners, 4 salaried GP's and an Advanced Care Practitioner. The Nursing team has 3 Practice Nurses and 3 HCA's

The practice uses Emis as a clinical system. We are paper light, so scan all incoming correspondence onto patients medical records.

Job Summary

The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. The focus of the role is the delivery of evidence-based practice for patients with long-term conditions and management and preventative nursing interventions to all patients. The post holder will work collaboratively with the whole general practice and attached teams to meet the needs of patients.

Key duties and responsibilities

Clinical practice

o Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions

o Review medication for therapeutic effectiveness

o Work with patients in order to support adherence to prescribed treatments

o Implement and evaluate programmes that promote health and well-being (e.g. smoking cessation and weight management)

o Implement and participate in vaccination and immunisation programmes for both adults and children

o Advise, support and where appropriate, administer vaccinations for patients travelling abroad

o Promote and deliver evidence-based wound care and for patients presenting with aural conditions

o Preparing and maintaining environments and equipment before, during and after patient care interventions, including assisting GPs during minor operations

Delivering a quality service

o Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)

o Produce accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures

o Prioritise, organise and manage own workload in a manner that maintains and promotes quality

o Deliver care according to the NSF and National Institute for Clinical Excellence (NICE) Guidelines and evidence-based acre

o In partnership with other clinical teams, collaborate on improving the quality of health care, responding to local and national policy initiatives as appropriate
