Job summary
The Coastal Partnership is looking for a passionate, motivated, and experienced ANP Prescriber to deliver exceptional clinical care to our patients across our four coastal sites: Martham, Hemsby, Ormesby, and Caister Surgeries.
Main duties of the job
The Successful candidate is required to practice independently, assess, diagnose, plan, implement and evaluate treatment and care for patients presenting with an undifferentiated diagnosis.
They should demonstrate safe clinical decision making, management, plan, prescribing and work with collaboratively with the practice team to meet the needs of the patients. they should have good experience assessment and management skills and will be supported by GPs and other clinicians.
GP experience or secondary care experience is preferred.
Advance Nurse Practitioner Qualification and independent prescribing is required.

Accurately examine and assess patients with an undifferentiated diagnosis, from physiological, psychological and social perspective, and plan clinical care accordingly.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
Ensure full and accurate documentation in patient notes of clinical history, physical examination, diagnosis and treatment plan.
Your work will be of a collaborative nature with our General Practice team to meet the needs of patients. The role will include:

Working with the triage team

Offer telephone and face-to-face same day appointments for acute cases.

See patients with minor illness and minor injuries.

Home visits and holistic reviews.

Care Home Ward Rounds
Be able to work independently and perform home visits
Have excellent clinical and time management skills
Be able to demonstrate critical thinking in the clinical decision-making process
Independently decide on and carry out treatment of minor ailments.
To attend mandatory training annually
