Job summary

At The Coastal Partnership we are excited to advertise for a Care Coordinator, as a Care Coordinator you will collaborate closely with the Practices multidisciplinary team to coordinate essential activities, including facilitating access to services, providing advice and information, and ensuring timely, efficient, and patient centred health and care planning.

Main duties of the job

In this role, you will assist patients in actively participating and connecting with daily life by implementing tailored activities aimed at enhancing, preserving, or restoring skills for individuals with cognitive, physical, or mental disorders, conditions or illnesses. Additionally, you will facilitate the continuity of care, serving as a primary contact for families, residents and professionals involved in the care of identified patients or specific patient group.

Job description

Job responsibilities

  1. Coordinate and guide patient through health and care services, collaborating closely with social prescribers, health and wellbeing coaches and the multidisciplinary team.
  2. Utilise data analysis and local patient insights to identify and address the needs of specific patient groups, delivery personalised care.
  3. Assist practice staff and patients in engaging in shared decision-making discussions, providing support tools, and aiding in understanding their health and wellbeing activation levels.
  4. Manage patient queries, appointment, and information dissemination to empower informed decision-making regarding care choices.
  5. Compile comprehensive care and support plans aligned with individual preferences and best practices.
  6. Represent the practice at relevant meetings, offering insights and feedback on care coordination priorities.
  7. Serve as the Practice Lead for vaccination initiatives, ensuring a coordinated approach across the multidisciplinary team.
  8. Support drug monitoring call and recall processes to uphold patient safety.
  9. Forge strong connections with local agencies, promoting equality and inclusion efforts.
  10. Engage directly with care homes and key providers, facilitating the exchange of pertinent information.
  11. Implement and adhere the safeguarding protocols for vulnerable individuals.
  12. Maintain accurate records of support activities, inputting data into clinical systems while complying with data protection regulations.
