The Coastal Partnership is well-regarded for GP, Student, and nurse training. We like to grow our own and support personal development and mutual learning. We enjoy regular clinical meetings, half day closures for protected learning time, and enable daily coffee meetings for clinical discussions and case sharing. We also provide active mentoring and support to help with our ongoing education and CPD.
Main duties of the job
We are purposeful in our efforts to manage GP workload and ensure sustainability, having developed a diverse and large team of support staff over recent years. A combination of the additional roles, Patient Services and Patient Administration teams provide significant support in managing the patient journey, including, but not limited to, processing incoming documents, enacting medication changes, and undertaking medication reviews. We have clinical support from a large nursing team and newly expanded Allied Clinical Professionals team.
The successful candidate will provide a full range of medical services as defined in the core GMS (General Medical Services) contract and additional enhanced services. These include telephone triage leading to remote or face to face consultations, prescribing medication. Work with the practice team to achieve enhanced and locally commissioned services and QOF targets. Participate in projects and research within the practice.

Our Partnership is led by 3
partners and a comprehensive management team; delivery is over a 7-site
footprint, spanning 3 PCNs, 3 Places and offers Village, Town and City
demographics. We are currently offering positions at our two City Sites as follows;
Old Palace/ Beechcroft Surgeries 6 sessions per week.
Windmill Surgery located in Wymondham 8 sessions per week.

In accordance with the Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including:
- Face-to-Face consultations.
- Prescribing, checking, and signing acute and repeat prescriptions
- Work with the practice team to achieve enhanced service and QOF (Quality Outcome Framework) targets.
- Completing paperwork and correspondence in a timely fashion.
- Participate in the practice monthly clinical governance meetings.
To keep excellent clinical records including the use of clinical coding.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation.
Assessing the health care needs of patients with undifferentiated and undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current practice disease management protocols, developing care plans for health.
Providing counselling and health education.
Referring to other care providers as appropriate.
Prescribing in accordance with the Practice/CCG prescribing formulary (or generically) whenever this is clinically appropriate.

Awareness of and compliance with all relevant Practice policies/guidelines, e.g., prescribing, confidentiality, data protection, health, and safety.
A commitment to life-long learning and audit to ensure evidence-based best practice.
