Job summary
We are creating a new role within the team for an Advanced Clinical Practitioner (ACP) who will work
autonomously within the multidisciplinary team to provide high-quality,
patient-centred clinical care. The candidate will also play
a role in service development, education, and quality improvement initiatives.
Main duties of the job
The main duties of the role will be to assess, diagnose, treat, and manage patients presenting with
undifferentiated and undiagnosed conditions, supporting safe and effective care
across the practice.

Diamond Medical Group was formed in November 2023 and is a 4 site 20,000 patient organisation. We have a large team of GPs (partners and salaried), Nurses, HCAs, Pharmacists and work closely with our local PCN and their staff also work within the team.

Key Duties & Responsibilities
Clinical Practice
- Independently assess, diagnose, treat, and manage patients presenting with acute, chronic, and complex conditions.
- Prescribe and review medication in accordance with local and national policy.
- Request, interpret, and act on diagnostic investigations.
- Provide evidence-based management plans, including referral to secondary care or community services when appropriate.
- Undertake home visits, telephone, and online consultations as required.
- Recognise and manage medical emergencies, escalating appropriately.
- Promote health education, disease prevention, and self-care with patients and carers.
Leadership & Teamwork
- Act as an expert resource and role model within the practice team.
- Support and mentor junior clinicians, nurses, physician associates, and students.
- Contribute to multidisciplinary team meetings and case discussions.
- Support service development in line with practice and PCN priorities.
Education & Development
- Maintain professional development in line with the NHS ACP framework.
- Participate in appraisal, clinical supervision, and peer review.
- Contribute to the training and education of colleagues, including GPs in training and student placements.
Quality & Governance
- Work within local and national clinical guidelines, protocols, and policies.
- Participate in audit, quality improvement projects, and significant event analysis.
- Support the practice in achieving QOF, IIF, CQC, and other regulatory standards.
- Maintain accurate, timely, and legally compliant clinical records

- MSc in Advanced Clinical Practice
- Independent prescriber qualification, V300

- PhD or further postgraduate study
