We are seeking compassionate, motivated, and customer-focused Care Navigators to join our team at James Alexander Family Practice.

We have full and part time opportunities available across both our sites.

As a Care Navigator, you will play a vital role in supporting patients with complex health needs by helping them access the right care and support at the right time. This is an exciting opportunity to be part of a forward-thinking GP practice committed to improving patient care and promoting health and well-being.

Main duties of the job

Call Handling: Answering patient calls, providing information, and directing them to the appropriate services or healthcare professionals.

Task Management: Managing and prioritising patient-related tasks and ensuring timely follow-up.

Admin Support: Assisting with administrative duties such as maintaining patient records and managing appointments.

Signposting: Guiding patients to appropriate healthcare services based on their needs.

Reception Duties: Supporting the reception team with patient check-ins, scheduling, and general office tasks to ensure smooth practice operations.

James Alexander Family Practice is a large Primary Care Organisation delivering high-quality General Practice services across two sites Bransholme Health Centre and Princes Medical Centre.

Patient-centred care is at the core of everything we do, and one of our key objectives is to consistently provide safe, high-quality care that meets the needs of our patients.

We care for a combined list of more than 21,500 patients across our two sites. Our multidisciplinary clinical team includes GPs, Clinical Practitioners, First Contact Physiotherapists, GP Assistants, Practice Nurses, Health Care Assistants, and a dedicated Mental Health Practitioner. We are also supported by an experienced in-house Pharmacy Team.

We work in close partnership with Marmot Primary Care Network, both contributing to and benefiting from the shared expertise, resources and support within the network.

  • Process personal, telephone and e-requests for appointments
  • Answer incoming phone calls, transferring calls or dealing with the callers request appropriately

  • Process incoming and outgoing mail
  • Initiating contact with and responding to, requests from patients, team members and external agencies
  • Photocopy documentation as required
  • Input data into the patients healthcare records as necessary
  • Scanning of patient related documentation and attaching scanned documents to patients healthcare records
  • Manage all queries (including administrative queries) as necessary in an efficient manner
  • Maintain a clean, tidy, effective working area at all times

  • The ability to work individually or as part of a team To be able to apply yourself in an appropriate manner in a potentially challenging environment.

Desirable

  • Experience of working in primary care desirable but not essential as full training will be provided.

Qualifications

  • GCSE grade A to C in English and Maths.

Desirable

  • Previous experience in a similar role would be advantageous but not essential as full training will be provided.
