Job summary
We are looking for an experienced Practice Nurse to join our Nursing Team and support the Practice in delivering high-quality patient care to our population.
Experience of working as a Practice Nurse in Primary Care is essential and finding the right person to complement our Team is important to us.
Main duties of the job
Deliver evidence-based care following NICE and NSF guidelines.
Support and promote patient self-care and adherence to treatment.
Provide chronic disease management (Diabetes, COPD, Asthma, Hypertension).
Carry out cervical screening, contraception reviews, health checks, and dietary advice.
Undertake injections such as B12, Depo-Provera, Zoladex
Manage vaccine stock, storage, and ordering processes.
Provide supervision, mentorship, and support to NAs, HCAs and new clinical staff.
Support the delivery and achievement of QOF targets, including data collection, clinical recall, and evidence-based care.
Participate in monthly QOF review meetings and contribute to improvements in chronic disease management.
Support the seasonal flu campaign, including patient recalls, vaccine administration, data recording, and staff coordination.
Assist in other immunisation and vaccination campaigns as required (e.g. COVID-19, shingles, pneumococcal).

We are a large Practice of 15000 patients in Sunderland
North. The Practice is part of the Deep End Network delivering care to the most
deprived in the area.
Our team consists of 3 GP Partners, Salaried GPs, ANPs, PNs, NAs, Health Coach, HCA, Phlebotomist, Clinical Pharmacist, Pharmacy Technician, the PCN Team and a large Admin Team. As a teaching Practice we host medical students, GP trainees, pharmacy students, pre-registration pharmacists, nursing students and admin apprentices.
