Job summary
We are seeking an experienced and motivated Lead Practice Nurse to join our primary care team. As Lead Practice Nurse, you will play a key leadership role in the delivery and development of high-quality nursing services within the practice.
You will be responsible for overseeing the nursing team, developing staff potential, and motivating your team through mentoring, appraisals and learning events. You will contribute to quality improvement initiatives and ensure clinical excellence.
This is a pivotal role that combines hands-on clinical practice and leadership. While previous leadership experience is desirable, we also welcome applications from experienced practice nurses who are ready to take the next step in their career. For the right candidate this can be a developmental role with support to transition into leadership.
Interviews will be held on the 3rd of July.
Main duties of the job
Provide clinical leadership & direction to the nursing team, ensuring high standards of patient care are consistently delivered. Supervise, mentor and support the practice nursing team, identifying training and support needs to support ongoing professional development.
Coordinate and lead the delivery of a wide range of nursing services, including chronic disease management (e.g. diabetes, asthma, COPD), immunisations, wound care, cervical screening, and health promotion.
Ensure compliance with current clinical guidelines, infection control policies, and national/local protocols. Contribute to audits & risk assessments
Work collaboratively with GPs, other clinical staff, and external health and social care partners to ensure integrated, high-quality care.
Support the development and implementation of practice policies, protocols, and procedures in line with best practice and regulatory requirements.
Take a lead role in managing and reviewing patients with long-term conditions, using data and recall systems to support proactive care.
Take a lead on contraception services, providing coils and Depo (Training can be provided). HRT reviews.

We have currently moved to a total triage model which is now well integrated into our practice.
