Job summary

Bridlington Primary Care Network (PCN) in partnership with Cancer Alliance and Macmillan Cancer Support, are looking for a highly motivated and knowledgeable Cancer Care Coordinator Administrator to join our dynamic and friendly team that is committed to delivering exemplary patient care for our patients.

Working collaboratively with Cancer Alliance and Macmillan the role has been purposely created to support and improve patient experience focusing on key areas to improve the cancer pathway and address some of the Health Inequalities experienced by patients.

The successful candidate will play a key role in proactively identifying and supporting patients both after diagnosis (such as delivering Cancer Care Reviews) and before diagnosis (supporting the uptake of the national cancer screening programmes). They will work closely with GPs and other primary care colleagues within the Primary Care Network (PCN) and working to improve the integration between Primary and Secondary Care.

The successful candidate will have excellent interpersonal and communication skills and be organised, patient and empathetic. They will have experience of working in health, social care or other support roles, including direct contact with people, families or carers.

Main duties of the job

Improved screening: Cervical, Breast, Bowel and Lung screening and the reduction of DNAs.

Focusing and working with patients that are not accessing the screening programmes.

Improve the patient experience by providing non-clinical delivery of holistic Cancer Care Review.

Undertake cancer awareness activities and actively participate in various events as the Cancer Care Coordinator or on behalf of Cancer Alliance/Macmillan.

Referrals to service to support cancer prevention and improve cancer outcomes.

Prehabilitation - identify patients that are about to start cancer treatment, ensuring they have the relevant support and understanding of the purpose of any treatment.

Support health and equalities by experience by patients including the Learning Disabilities (LD) population.

Humber Primary Care and Drs Reddy and Nunn are both teaching practices linking into academic partners to provide ongoing support and development to the workforce.
