Job summary

An exciting opportunity has arisen for a COPD nurse to join the Bridlington Primary Care Network. Our dynamic and friendly team is committed to delivering exemplary patient care for our patients.

The post is a 12 month fixed term contract / secondment opportunity.

Main duties of the job

The aim of the COPD project is to reduce hospital admissions and multiple admissions for COPD patients in Bridlington. The post is ideal for someone looking to consolidate and further develop their skills in respiratory care.

You will proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and improve patient outcomes. Working autonomously, you will have continuing responsibility for proactively monitoring and managing patients caseloads.

You must be able to demonstrate the ability to work as an effective member of a multidisciplinary team, having excellent clinical skills to deliver exceptional standards of care for all patients and their families.

You must be well organised and enthusiastic in participating with service development through audit and peer review. Helping to drive and sustain new initiatives to meet local and national standards.

Bridlington PCN is a forward-thinking network comprising of 2 practices over three sites within close proximity to each other with a total patient population of 42,000. Humber Primary Care and Drs Reddy and Nunn are both teaching practices linking into academic partners to provide ongoing support and development to the workforce.

Our aim is to improve care for patients by working collaboratively across primary care and our partners as part of the integrated care partnership.
