We have an exciting opportunity for an enthusiastic and committed Practice Nurse to join our friendly team. Previous experience within Primary Care is essential.

Main duties of the job

The successful candidate will work collaboratively with our multi-disciplinary general practice team to meet the diverse needs of patients. A long term passion and commitment to nursing within General Practice is essential.

Our collaborative clinical team includes 18 GP's, 5 practice nurses, healthcare assistants, ANP's, paramedics, and a dedicated pharmacy team.

Job description

Job responsibilities

Meeting patients nursing care needs as identified, or where care has been transferred to them by a GP.

Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary.

Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs.

Responsible for ensuring the effective and timely delivery of a selection of the following clinics:

  • Asthma
  • Child and Adult Immunisations
  • Diabetic reviews
  • Hypertension Clinic
  • IHD Clinic
  • COPD Clinic
  • Flu Vaccination Clinic
  • Cervical Smears
  • Family Planning
  • Warfarin monitoring
  • Wound management including leg ulcers
  • Removal of foreign bodies
  • ECGs

It is essential you are registered with the Nursing and Midwifery Council (NMC).
