Main duties of the job
The post holder is responsible for the delivery of
general practice nursing to the whole practice population. The role encompasses
two main roles; the delivery of evidence-based practice for patients presenting
with a long term condition and the provision of preventative health care to the
practice population.
As an autonomous practitioner the nurse is responsible
for the care delivered and must demonstrate critical thinking and skills in
clinical decision making when managing patients. The practice nurse will work
collaboratively within the general practice team to meet the needs of patients
and support the delivery of policy and procedures.
About us
Our mission is to provide better health for our patients by improving outcomes; improving patient access to services; providing quality care closer to home where possible; motivating and developing our workforce; maximising limited NHS resources to provide the best care and support.
Our core values that are shared among the partners and staff are:
- Professionalism
- Compassion
- Fairness
- Respect
- Accountability
The Practice has always prided itself on providing high quality care in a traditional family practice setting. We have built a positive relationship with our consultant colleagues at UHD. Two of our partners are GP trainers, and learning and education is an important part of what we do. One of our partners is Executive Lead for Integrated Care at UHD and we are heavily involved in network development.
We are a SystemOne practice with approximately 9500 patients, based in purpose built premises.
