The successful candidate needs to be focused driven, have a caring and empathetic approach as this post requires the right individual to act as advocate. You must also be able to demonstrate excellent communication skills and have the ability to reach out to those hard to reach patients who do not engage or take responsibility in their health and social care needs ensuring you use the available resources and sign post them to the appropriate services
You must have good IT and communication skills, and have the ability to adapt with care and empathy to patients individual needsto ensure we deliver the best standards of care to our Practice population.

Main duties of the job
Objectives
of the role
The objective of the care coordinator is to play a key role in proactively identifying and working with people,
including the frail/elderly with long-term conditions, to provide coordination
and navigation of care and support across health and care services.
The
Practice Care coordinator will provide senior support to the practice team in
targeting hard to reach groups with a focus on inequalities including Core20
Plus 5 groups and patients
The
Care Coordinator will review peoples identified needs and help to connect them
to the services and support they require, whether within the practice or
elsewhere for example, community and hospital-based services.
Working
closely with social prescribing link workers, health and wellbeing coaches, and
other primary care professionals team to provide an all-encompassing approach to personalised care and
enable people navigate through the health and care system.
Identifying
target areas such as long term conditions, frailty, DES, QOF and IIF targets.
Support practices in achieving their recall targets and providing an enhanced
outreach service to those hard to reach groups.
Identifying
patients requiring proactive support ie hard to reach patients, high risk
patients as identified in MDTs/practice meetings.

We are a busy Practice on the outskirts of Sunderland City Centre the Practice is situated in an area where health inequalities and deprivation are at there highest compared to other local areas.
We currently have a list size of 5345 patients.
Our friendly team are very diverse, always remain professional and continue to deliver excellent standards of care despite a very fast pace setting where we have higher than most health inequalities and deprivation . We ALL know the importance of team work, we all take pride in supporting one another to enable us to achieve our optimum goal in the delivery of good standards of care.
We have a team of 4 GP's 1 ANP, 2 PN, Health Care Assistant, Practice Pharmacist, Pharmacy Technicians, Social Prescribing Team, Mental Health Nurse and of course a very well supportive Administration Team.
