Practice Nurse
Job Purpose:
To provide a high quality care service based on the needs
of the Practice population which includes treatment, screening, preventative
care and patient education
Clinical
To manage patients with Long Term Chronic Disease, running specific clinic sessions and
maintaining robust recall systems and accurate recall date entry
Carry out Cervical Smear Sampling and attend annual
Cervical Cytology updates
Provide Travel Health advice and administer appropriate
vaccinations
Undertake all Child Health Surveillance and Immunisation
Vaccination Programmes
Provide directly and support our patients with
contraceptive services Short acting and Long acting medications
Provide ongoing support and have an empathy to
patients/carers /parents Palliative
needs
Undertake health promotion to help educate patient
Select, carry out and interpret results of specific test
Support inequalities and deprivation
Administration
To maintain accurate patient records and enter onto the
computer using agreed Read Codes
General
To maintain own annual registration with the NMC and adhere
to Code of Professional Conduct

Main duties of the job
> To have the ability to adopt a holistic approach in the delivery of good standards of patient care, providing management and treatment plans for ALL long term chronic conditions.
> To promote and administer vaccinations to all National & Local Screening and Vaccination Programmes
> To promote National Cancer Screening programmes
> Child Health Surveillance Childhood Vaccination Programme
> Carry out Cervical Smear Sampling
> Weight Management Programmes referring to appropriate weight Management Service Providers
> All Health & Well-Being related services smoking and alcohol advice advice to patients relating to the implementation & support of appropriate pathways
> Respiratory Conditions carry out Spirometry and FENO testing
> Blood Sampling for Long Term & Acute Conditions
About us
We are a friendly Practice situated on the outskirts of Sunderland City Centre. We currently have a list size of approximately 5434 patients.
We have a full complimentary Team of 4 GP's 2 Practices Nurses 1 Health Care Assistant and 1 Phlebotomist The Clinical Team are supported by a great and very friendly Administration & Management Team.
We are rated good overall by the CQC.
National and Local patients Satisfaction Surveys mark us at 98% overall patient satisfaction whereby the patients have indicated they are satisfied with the care and service they have received by out Team.
We offer ample opportunities for role progression and fully support training & development.
We excel in terms of friendliness and support to one another we have a family orientated approach although always mindfull and maintaining a good level of professionalism to our fellow colleagues and patients.
