Job summary

Experienced Practice Nurse who is passionate about supporting patients living with a long-term condition.

24 hours per week, over three days.

Main duties of the job

Offering care, support and education to patients we are committed to:

  • Continuity of care
  • Looking after each other as well as our patients
  • Supporting future nurses and GPs through hosted placements
  • Maintaining strong patient satisfaction ratings

A minimum of two years post-registration experience as a Practice Nurse providing care to patients with long-term conditions.

About us

Based at Truro Health Park, we look after over 18000 patients in Truro and the surrounding area.

You will be part of a dedicated team of GPs, paramedic practitioners, nurses, HCAs and physiotherapists.

Experience

Essential

  • Experience of working as a Practice Nurse.
  • Post registration experience in managing long-term conditions e.g diabetes, preferably at Band 6.
  • High level of clinical skills include cervical cytology, family planning/women's health, immunisation and vaccination, wound management.
  • Well-developed communication skills in English, both written and verbal.
  • Good IT skills.

Qualifications

Essential

  • Nursing degree qualification with full registration of NMC.
  • Qualification in long-term conditions.

Knowledge

Essential

  • Knowledge of needs of patients with long-term conditions.
  • Knowledge of health promotion strategies.
  • Awareness of clinical governance issues in primary care.

Desirable

  • Knowledge of public health issues in the local area and wider health economy.
