Job summary

We have an exciting opportunity due to a member of our team relocating to Scotland, for an experienced, highly motivated practice nurse to join our dynamic nursing team, which consists of one Practice Nurses and two Nurse Associate. The post holder will use their nursing skills to deliver the highest standards of nursing care in accordance with evidence-based practice and national practice protocols. We would encourage the successful candidate to lead on diabetes and areas of patient care that are of interest to them, appropriate to the role and that fit in with the Practice's requirements and vision.

Previous GP Practice Nursing experience is essential.

Working pattern and hours are negotiable.

We are a team of 2 GP partners, 3 GPs, 1 Practice Nurses, 2 Nurse Associate, 2 pharmacist plus a team of 12 admin and clerical staff members working alongside in-house PCN staff, with approximately 5400 patients. In partnership with our patients, we aim to provide high quality flexible patient care sensitive to the individual needs of all patients.

Main duties of the job

Main duties of the job include:

To practice autonomously without direct supervision.

Develop an understanding of general practice care delivery standards, inclusive of QOF points and support the Practice to achieve these.

To lead on diabetes.

To utilise clinical skills in long term chronic disease management including asthma, CHD, CKD, diabetes, dementia, hypertension and respiratory

Immunisations

Wound care management including dressings, suture removal and ulcer care.

ECG recording

Performing diagnostic procedures including BP, peak flow, and urine testing

Spirometry recording and interpretation.

Cervical cytology

Contraceptive and sexual health advice

NHS Health checks

Health promotion

Travel health

Ensuring safe storage, rotation and disposal of drugs and vaccines

Good communications with other team members when clinical findings on routine examination indicate possible action / follow up.

To participate in clinical meetings

To promote the protection of children and vulnerable adults in accordance with local safeguarding procedures.

To adhere to the Infection and Control policy for the service being provided.

To assess and evaluate outcomes of own practice, service intervention and treatment and if identifying any short falls to report these to the Practice Manager
