Job summary
The Practice Nurse is to support new-to-practice
nurses and other healthcare professionals within the organisation and across
the wider network to enhance integration with community, secondary, and social
care services.
This
organisation must ensure that the post-holder has access to appropriate
clinical supervision and an appropriate named individual in the organisation to
provide general advice and support on a day-to-day basis. Further information

Main duties of the job
- Deliver high-quality nursing care to patients, including wound management, immunisations, cervical screening, and long-term condition reviews (e.g. asthma, COPD, diabetes).
- Support chronic disease management through patient education, treatment plans, and regular health monitoring.
- Undertake health promotion, lifestyle advice, and preventive care such as NHS Health Checks and vaccinations.
- Assist with treatment room duties including ECGs, blood pressure monitoring, ear care, injections, and sample collection.
- Maintain accurate clinical records and ensure care is delivered in line with current evidence-based practice and clinical guidelines.
- Work collaboratively with GPs, healthcare assistants, and the wider multidisciplinary team to provide safe, effective patient care.
- Participate in safeguarding, infection control, medicine management, and contribute to QOF, enhanced services, and practice audits.
- Support patients with complex needs and provide continuity of care where appropriate.
