Job summary

We are looking for a motivated and detail-oriented Pharmacy Technician to join our vibrant, forward-thinking team at Chiswick Medical Practice.

The Pharmacy Technician will play a key role in supporting safe, effective, and efficient medicines use across the practice. Working alongside our Clinical Pharmacists, GPs, and wider multidisciplinary team, you will help deliver high-quality medicines management and optimisation to improve patient care and ensure best use of NHS resources.

Main duties of the job

The Pharmacy Technician will support safe, effective, and efficient medicines use across the practice. Duties include managing repeat prescriptions, conducting basic medication reviews under pharmacist supervision, and supporting medicines optimisation and formulary compliance. You will perform medication switches or adjustments, assist with patient and prescriber queries, and co-ordinate medication reviews.

You will undertake medicines reconciliation after hospital discharge, liaise with community pharmacies and hospitals, and ensure accurate record keeping.

The role also involves supporting MHRA alerts, audits, and safety initiatives, providing patient education on medicines use, and contributing to quality frameworks such as QOF and IIF. You will work collaboratively within the multidisciplinary team to enhance prescribing safety and patient care.

Job responsibilities

Main duties of the job1. Medicines Management

  • Support and manage repeat prescription processes, ensuring accuracy, safety, and cost-effectiveness.

  • Conduct basic medication reviews under the supervision of a Clinical Pharmacist.

  • Support medicines optimisation activities and implement formulary or prescribing policies.

  • Participate in audits, incident reporting, and continuous quality improvement initiatives.

  • Utilise GP practice clinical systems (e.g., EMIS) to conduct prescribing audits.

  • Participate in practice and multidisciplinary meetings to promote safe, effective prescribing.

4. Patient Support and Communication
  • Provide education and advice to patients about safe and effective use of medicines (within scope of practice).

  • Signpost patients to appropriate services or self-care options where suitable.

  • Support initiatives to improve patient understanding and adherence to prescribed treatments.

5. Practice Development

  • Contribute to the development and improvement of medicines management systems within the practice.

  • Support QOF, IIF, and other contractual or quality frameworks related to prescribing.

  • Participate in multidisciplinary team meetings and share learning with colleagues.
