Job summary

The Practice Nurse will provide care and demonstrate a safe and high professional standard of nursing care for patients within general practice. They will work collaboratively with the general practice team to meet the needs of patients and should have the ability to prioritise, be flexible and responsive to patient needs. The support and delivery of practice policies and procedures is essential. The role of the practice nurse includes duties such as cervical screening, chronic disease management such as diabetes and respiratory reviews. Childhood immunisations and travel vaccinations. Managing anticoagulation clinics, venipuncture hypertensive management and contraception reviews.

Main duties of the job

We are looking for someone to join are ever expanding nursing team. The ideal candidate will be someone who is a team player but also can work independently. Is enthusiastic and wanting to improve the health of our patients. The ideal candidate will also strive to work towards Holderness Health values. The candidate will need to be a qualified nurse ideally with practice nurse experience, but if not experienced or qualified in some areas training and support can be given.

We are a large rural practice with approximately 34,000 patients. As a single-practice Primary Care Network, we have a wonderful opportunity to transform care for our patients. We operate from 7 locations across Holderness and you must also be willing to travel between sites as necessary.

Our hard-working and dedicated team includes 23 GPs, an extensive multi-disciplinary team of healthcare professionals and a great patient services team.

JOB TITLE: PRACTICE NURSE

REPORTS TO: LEAD NURSES

Job Summary

The Practice Nurse will provide care and demonstrate a safe and high professional standard of nursing care for patients within general practice. They will work collaboratively with the general practice team to meet the needs of patients and should have the ability to prioritise, be flexible and responsive to patient needs. The support and delivery of practice policies and procedures is essential.

Job Responsibilities

Assist in and perform tasks related to patient care as directed by senior nursing staff and GPs. Tasks will only be delegated in line with level of training/experience and full training will be provided when required to carry out essential roles.

Tasks that might be performed by a practice nurse depending on experience include:

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care and minor ailments with referral to GPs as necessary.

Cytology and sexual health screening

Assist in managing and monitoring practice cervical cytology failsafe system

Prescribing for patients from Nurse Formulary

Chronic Disease management eg care of patients with Diabetes,CHD,COPD/Asthma

Multimorbidity clinics

Joint clinics with specialist nurses eg Diabetes Specialist nurse

Contraception advice.

Wound care / ulcer care/Doppler

Assisting in running INR Clinic near patient testing and dosing using INR Star

Removal of Sutures

New Patient Medicals/Urinalysis

Travel Clinic

Asthma/Peak Flow Readings/Spirometry

Venepuncture

Actioning & managing results

Hypertension Management

Ear syringing

Routine immunizations/Childhood immunisations

Chaperoning and assisting patients where appropriate who are being examined by another clinician

Maintaining and cleaning equipment used by the nurses and GPs

Undertake housekeeping duties including, general tidiness and cleanliness of nurses and treatment rooms

Ordering of vaccinations to maintain stock levels

On occasion there may be a requirement to undertake home visits

Additional appropriate tasks that might be identified from time to time
