Job summary
We are looking for a hard working and customer focused Care Navigator and experienced Correspondence Management clerk to join our friendly practice family.
Candidates must be able to work in a highly demanding fast paced environment and work well under pressure.
This position would especially suit an applicant with previous experience in a primary care setting with an interest in maintaining excellent patient relationships.
Main duties of the job
Care Navigation involves daily face to face contact with patients, arranging of appointments and follow up of patient related tasks.
The aim of the role is to support and navigate patients to the most appropriate care for their needs.
Candidates must have excellent communication and computer literacy skills are essential.
Managing clinical correspondence received by the practice. Including readcoding into patient records.

- To have a thorough knowledge of all Practice procedures
- To work in accordance of written protocols
Care Navigator
- Receiving patients consulting with members of practice team
- Be able to cover all reception positions as necessary
- Explain procedures to patients i.e. how to register, Immediate
& Necessary Treatment etc
- Keep abreast of relevant topics and the latest guidance therefore
giving patients the latest and best information
Appointments
- Process appointment requests
- Deal with visits requests (see protocol)
Confidentiality:
- In the course of seeking treatment, patients
entrust us with, or allow us to gather, sensitive information in relation
to their health and other matters. They do so in confidence and have the
right to expect that staff will respect their privacy and act
appropriately
Correspondence Management
- Scanning correspondence onto the clinical system following agreed protocols
- Identify the correspondence and readcode into the patient records accurately
- Complete correspondence management document tasks accurately and in a timely manner
- Action any Subject Access Requests (SARs) by using the iGPR system
- Working closely with the Data Quality and reception team
