Job summary

Horizon Healthcare is offering a 12-month fixed-term maternity cover position, with the exciting possibility of a permanent post for the right candidate.

  • No on-call commitment
  • No Acute visits
  • On-call Partner support throughout the whole day
  • A Clinical Pharmacy team who undertake all prescription tasks and all medication related correspondence and Patients with Long Term Conditions
  • Mental Health Professionals who are on site, managing MH consultations

  • Additional support of associated allied health professionals, such as First contact physio and dieticians, social prescribing and care coordination.

Main duties of the job

To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract. Furthermore, the post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.

You will be joining a forward-thinking General Practice with motivated multi-disciplinary teams of both administrative and healthcare professional colleagues. Horizon Healthcare operates a "Primary Care Consultant" model for our team of GPs giving value to the individual and propelling them into a learning environment where they will have the constant support of an on-call partner. Administrative duties are considerably reduced by our other in-house specialist teams. Horizon Healthcare has a list size of 27,000 plus patients and offer competitive rates of pay based on experience and skill levels.

Job responsibilities

The following are the core responsibilities of the salaried GP. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. The delivery of highly effective medical care to the entitled population

b. The provision of services commensurate with the GMS contract

c. Generic prescribing adhering to local and national guidance

d. Effective management of long-term conditions

e. To clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly referring patients to other services/agencies in a timely manner, utilising Practice and any other local guidelines

f. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

g. To support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care

h. Processing of administration in a timely manner, including referrals, repeat prescription requests and other associated administrative tasks

i. Review medications following the appropriate policies, NICE/local clinical guidelines and local care pathways

j. Support the delivery of anticipatory care plans

k. On a rotational basis, undertake telephone triage and duty doctor roles

l. Maintain accurate clinical records in conjunction with good practice, policy and guidance

m. Working collaboratively, accepting an equal share of the practice workload

n. Adhere to best practice recommended through clinical guidelines and the audit process

o. Contribute to the successful implementation of continuous improvement and quality initiatives within the practice

p. Accept delegated responsibility for a specific area (or areas) or the QOF

q. Attend and contribute effectively to practice meetings as required

r. Contribute effective to the development and maintenance of the practice including; clinical governance, training, financial management and HR

bb. To be an integral part of the general practice team as well as supporting the wider network and multi-disciplinary team

a. Participate in practice audits as requested by the audit lead
