Job summary
An opportunity has arisen for an enthusiastic, well organised, experienced nurse to join our friendly and supportive team to provide a high standard of service to our local population.
We perform well, both in terms of Bolton Quality Contract and QOF outcomes, and provide a wide range of enhanced services.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available.
Main duties of the job
- Conduct patient health reviews for Long Term Conditions, development of individual patient centred management plans at annual review; agreeing these as appropriate with the patient and other health professionals in accordance with current guidelines and with knowledge of QOF requirements and NICE guidelines
- Performing medication reviews and assessments for long term conditions
- Educate, advise and support newly diagnosed and established chronic disease management patients
- NHS Health Checks/New Patient Checks
- Assist at IUD fitting clinics
- Testing Urine samples
- Blood results reporting (training and protocols provided)
- Assessment and administration of travel vaccinations
- B12 injections
- Depo administration
- ECGs
- Smears
- Pill checks/HRT checks
- Ordering & Administering appropriate immunisation and vaccinations
- Communicating with Child Health and D of H on immunisation queries and programmes
- Maintenance of cold chain and fridge temperatures
- Collaboration of equipment
About us
We are a training practice consisting of 7 GP partners, 2 Salaried GPs, 2 ANPs, GP Trainees, Paramedics, Pharmacists and Practice nurses. Our list size is approximately 11,000.
