Job summary

The individual will be responsible for the effective and efficient administration and processing of patient data, through optimising coding and analysis processes to support clinical functions and maximising attainment of Quality Outcome Framework (QOF), Enhanced Service contracts and immunisation and screening programs for Oaks Healthcare and support the IIF PCN Programme.

The individual will be responsible for directly engaging with patients to understand the challenges the patients may face when not engaging with QOF, long term condition recalls, immunisation programs or any other recalls relevant to patients age or condition, with the aim of improving the outcome for the patient concerned.

Main duties of the job

To co-ordinate and administer effective administration processes to engage with targeted patient groups (e.g., patients with chronic diseases)

To generate invites and bookings for vaccinations, health checks, screenings, smears, chronic disease reviews and support the communication of various health campaigns.

Provide clinical data analysis, audit and quality control support to improve patient outcomes whilst developing and maintaining data entry templates, carrying out searches within EMIS and inputting data into CQRS.

Coordinate activity supporting QOF, Enhanced Services and Research Projects; this will include checking data as necessary and liaising with the practice teams to improve performance/uptake.

Assist in producing information for clinical audit research projects and screening initiatives.

Support seasonal and other annual health campaigns to increase patient uptake of services.

Undertake all necessary contract administration to ensure that the Practice maximises its income from the Quality and Outcomes Framework (QOF) and enhanced services contracts.

Oaks Healthcare is a large GP practice situated in Waterlooville.

We have over 16,000 patients, with 9 GPs ands 4 ANPs as well as other clinical staff.

We have a large friendly admin team who support the running of the practice.

  • Educated to NVQ 3 level or relevant equivalent experience.
  • GCSE Mathematics and English level 4 or above or equivalent.
  • ECDL or equivalent IT skills and knowledge.
