This is an exciting role for a motivated and compassionate nurse who wants to make a real difference. You will work collaboratively within a multidisciplinary team to deliver a wide range of clinical services, including chronic disease management, immunisations, cervical screening, wound care, and health promotion, while developing meaningful relationships with patients.

Main duties of the job
Working Pattern:
- Monday 8.30am to 6.00pm
- TuesdayOFF
- Wednesday 8.30am to 6.00pm
- ThursdayOFF
- Friday 8.30am to 6.00pm
30 days of annual leave + Bank holidays.
As a Practice Nurse, you will deliver high-quality, patient-centred care across a range of clinical areas within general practice. Your day-to-day work will include supporting patients with long-term conditions such as asthma, diabetes and cardiovascular disease, carrying out routine reviews, and promoting self-management and healthy lifestyles.
You will be responsible for providing core nursing services including immunisations, cervical screening, wound care, and health checks, as well as supporting infection prevention and control within the practice. You will work closely with GPs, healthcare assistants and the wider multidisciplinary team to ensure safe, effective and coordinated care for patients.
The role also involves maintaining accurate clinical records, supporting recall and screening programmes, and contributing to quality improvement initiatives such as QOF. You will play an important part in ensuring patients have a positive experience of the practice, offering compassionate care and clear communication at all times.

The Practice Nurse will support the delivery of high-quality, safe and effective care to the registered patient population, working within professional competence and in line with national and local guidance.
Key responsibilities include:
Clinical Care & Patient Management

Undertake consultations and assessments of patient need, including patients and their families

Deliver evidence-based nursing care in line with NICE, national service frameworks, and local protocols

Support the management of long-term conditions, contributing to quality improvement and assurance initiatives

Provide wound care, including ulcer management and Doppler assessments
