Job summary

We are seeking an experienced and highly skilled Advanced Clinical Practitioner (ACP) to join our multidisciplinary team.

This is a senior clinical role for a practitioner who is able to work autonomously, using advanced clinical knowledge and judgement to assess, diagnose and treat patients.

The ACP will manage patients with both acute and long-term conditions, helping to reduce pressure on GP services and ensuring patients receive timely, effective care.

The role also includes undertaking home visits where clinically appropriate and supporting the supervision of students where appropriate.

Main duties of the job

As an Advanced Clinical Practitioner, you will work autonomously to assess, diagnose and treat patients with a wide range of acute and long-term conditions.

You will run your own clinics, carry out home visits when required, and support timely access to care by managing patients who would otherwise need a GP appointment.

You will play a key role in improving patient outcomes through high-quality clinical decision-making, health promotion and preventative care. Alongside clinical work, you will support service development, contribute to quality improvement, and provide supervision to students and developing staff where appropriate, helping to build a strong and skilled clinical team.

  • Registered healthcare professional with a recognised professional body
  • Educated to Masters level in Advanced Clinical Practice in line with the Multi-professional Framework for Advanced Clinical Practice in England
  • Evidence of working at an advanced level of practice
  • Qualified independent prescriber
  • Evidence of relevant professional and academic qualifications
  • Eligible for professional revalidation

Desirable

  • Advanced Life Support (ALS) and/or Paediatric Life Support (PALS)
  • Teaching, mentoring or supervision qualification
  • Evidence of specialist clinical training or extended skills relevant to primary care
