Job summary

The Dove Medical Practice has a fantastic opportunity for a motivated, and forward-thinking Advance Nurse/ Care Practitioner to join our established and respected medical practice.

This is flexible role and any sessions would also be considered. GP practice experience is preferable, however we would consider an experienced nurse with prescribing qualifications from A&E, Walk-in centre and out of hours. We encourage staff development & support training.

Salary is dependent on experience & qualifications and the role includes NHS pension, 6 weeks annual leave pro-rata.

We are looking for the right individual with experience in urgent care, minor ailments and injuries, who is confident in working independently yet able to work collaboratively with other members of the primary care team. As an Advanced Nurse/Care Practitioner, you will manage a clinical caseload, dealing with presenting patients needs in a primary care setting and assist in managing the home visit requests. You would act within your professional boundaries and with a holistic approach and be supported by the wider clinical team.

Main duties of the job

Main duties of the job

  • A Nurse Practitioner working independently within their professional scope and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.

  • Work with the team to develop new and innovative concepts, models, methods, and practices, to deliver primary care services to meet the needs of the practice

  • An element of the job may include visiting patients in their own home.

  • Implement and evaluate individual specialised treatment plans for chronic disease patients

  • Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

  • Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

  • Process and interpret pathology and other test results as required

About us

The Dove Medical Practice is located in Erdington/New Oscott and we are a large, stable GMS practice, based in a modern purpose-built premises, with a list size of 10,000.

We are a forward-thinking, training and teaching practice using EMIS Web, Docman, and AccuRx clinical systems.

We have a supportive & cohesive team with excellent reception, admin and management staff. We are part of KEN PCN and have several support staff including pharmacists, dietitians, physiotherapists and physician associates.

We have

Weekly Clinical; Meetings and are part of local PCN and a training practice, providing minor surgery and family planning services. We also participate in research and development.

  • Registered General Nurse or HCPC (Currently registered with the Nursing & Midwifery Council or HCPC)
  • Evidence of current NMC/HCPC registration
  • Educated to BSc level or equivalent experience
  • Accredited Advanced Nurse / Paramedic Practitioner Programme
  • Nurse Independent Supplementary Prescribing V300 / Non Medical Prescribing qualification

Desirable

  • Educated to Masters level or equivalent experience
  • Leadership qualification
  • Mentoring qualification
