Job summary

The post holder will work as part of a multi-disciplinary team in a patient-facing role.

Proactively manage patients with co-morbidities and provide structured medication reviews to optimise treatment outcomes.

Provide support to general practice staff with prescription and medication queries. Support the repeat prescription system, process acute prescription requests, and carry out medicines reconciliation following transfer of care, ensuring safe and effective prescribing practices.

Provide expertise in clinical medicines advice while addressing both the public health and social care needs of patients within GP practices.

Support medicines optimisation initiatives and contribute to quality improvement activities, including involvement in quality and outcomes framework (QOF) and enhanced services where appropriate.

Act as a resource for medicine-related queries, signpost patients to appropriate services, and liaise with other healthcare professionals to ensure coordinated and patient-centred care.

Main duties of the job

Hold patient facing comprehensive and complex reviews and provide medicine support

Manage medicines in discharges from hospital

Manage and assist the pharmacy team/Prescribing clerk pre-reg pharmacists

Hold long-term conditions management within general practice: e.g. Diabetes Hypertension, respiratory medicine

Provide patient telephone support

Be a centre of medicines information for practice staff

support and adhere to repeat prescribing processes

Complex Diabetes management including insulin initiation and titration

About us

We are a well-established GP training practice providing primary care services to approximately 14,000 patients across two sites. Our clinical team consists of four GP partners, three salaried GPs and two regular locum GPs, supported by a skilled pharmacy team including 2 pre-registration pharmacist. This multidisciplinary workforce enables us to deliver safe, effective and high-quality care to our practice population.

As an accredited GP training practice, we support the development of future clinicians by hosting medical students and GP trainees. Education, clinical supervision and continuous professional development are embedded within our culture, ensuring care is evidence-based and aligned with best practice guidance.

Our wider team includes two experienced practice nurses, three healthcare assistants and a dedicated, highly efficient administrative team who ensure the smooth day-to-day running of the practice and positive patient experience across both sites.

We operate a robust on-call triage system whereby all same-day requests are assessed by the duty GP, ensuring patients are prioritised according to clinical need and directed to the most appropriate clinician or service. This structured approach supports patient safety, effective demand management and good access.

We were rated GOOD at our most recent CQC inspection and are recognised as a high-achieving, well-led practice committed to continuous improvement, teamwork and compassionate, patient-centred care.
