Job summary

Ilex View Medical Practice in Rawtenstall are looking for an enthusiastic and highly motivated compassionate Advanced Nurse Practitioner or Paediatric Advance Nurse Practitioner to join our team. The post holder will join our well-established team to deliver a high standard of care to all our patients and families.

This GP practice serves over 9000 patients across Rossendale. We are looking for a proactive and highly motivated person to become part of our friendly and excellent team.

The successful applicant will be encouraged to take an active role in the leadership and development of the practice, incorporating all the aspects of being an ANP/PANP. They will be required to implement and drive improvements and initiatives.

The individual should enjoy working as part of a supportive team, possess good communication skills and have good general IT skills. Experience of using the clinical system EMIS is desirable.

Flexibility, compassion and a "can do" approach is essential for this position.

You will be well supported by the Surgery's GPs, ANP's, Practice Nurses, Practice Manager, Pharmacist's, Reception team and Administrative staff.

Main duties of the job

  • The successful candidate will be practicing autonomously as an ANP/ Paediatric Advanced Clinical Practitioner within the Practice, providing evidence based and high quality patient-centred quality care to children/adults presenting with a broad range of conditions, including both illness and injury.
  • Will use collaborative and independent judgement to assess, investigate, diagnose, plan, implement, manage and evaluate clinical care and management of children.
  • Will support an efficient and patient focused service
  • Will work within the multidisciplinary team to deliver advanced clinical practice, clinical leadership and provide supervision, support, structured teaching and mentorship for multidisciplinary professionals, to facilitate ongoing education, training and development.

To assess patients with undifferentiated undiagnosed problems.

To take responsibility for making autonomous decisions in relation to patient care, where they are trained and deemed competent to do so.

Using advanced assessment skills of history taking and clinical examination, make a diagnosis and initiate an appropriate care plan including any necessary diagnostic tests of referral

To promote health and screening in relation to primary prevention. Gain an understanding of chronic disease management and access training as appropriate

To use PGDs within current parameters according to the local prescribing guidelines.

To participate in the development and implementation of service/ICB guidelines and protocols.

To provide specialist advice on patient care to the healthcare team taking into account all aspects of clinical governance, and act as a specialist resource within the practice.

To work collaboratively and strategically with representatives of other clinical areas, disciplines and services to provide a seamless pathway of care.

To maintain good working relationships and communication with the multidisciplinary tea, other departments and agencies to ensure that efficient and individualised patient care is provided.

Attend training events to maintain professional development.

To attend any necessary training that is required to increase skills and knowledge, to provide safe and effective patient care.

To complete the non-medical prescribing module at university if necessary.

To ensure that accurate documentation and records of patient care are kept at all times.

Ensure confidentiality is maintained at all times as per Caldicott requirements
