1. Operational/ Main Responsibilities

To assess patients with undifferentiated undiagnosed problems.

To take responsibility for making autonomous decisions in relation to patient care, where they are trained and deemed competent to do so.

Using advanced assessment skills of history taking and clinical examination, make a diagnosis and initiate an appropriate care plan including any necessary diagnostic tests of referral

To promote health and screening in relation to primary prevention. Gain an understanding of chronic disease management and access training as appropriate

To use PGDs within current parameters according to the local prescribing guidelines.

To participate in the development and implementation of service/ICB guidelines and protocols.

To provide specialist advice on patient care to the healthcare team taking into account all aspects of clinical governance, and act as a specialist resource within the practice.

To work collaboratively and strategically with representatives of other clinical areas, disciplines and services to provide a seamless pathway of care.

To maintain good working relationships and communication with the multidisciplinary tea, other departments and agencies to ensure that efficient and individualised patient care is provided.

Attend training events to maintain professional development.

To attend any necessary training that is required to increase skills and knowledge, to provide safe and effective patient care.

To complete the non-medical prescribing module at university if necessary.

To ensure that accurate documentation and records of patient care are kept at all times.

Ensure confidentiality is maintained at all times as per Caldicott requirements

Understand internal and external factors that could affect the service.

Identify and determine areas within the service that could be improved.

Develop plans and implement creative solutions for the service.

Develop goals and implement a process for these to be achieved. Use critical thinking to solve problems and overcome obstacles

Develop action plans to achieve goals within an agreed timeframe

3. Leadership

Take a lead role to provide high quality patient care.

Contribute and access clinical supervision.

To have good self-awareness

To have excellent communication and interpersonal skills

To have respect for others in the team and the external stakeholders

To develop evidence based policies and procedures specifically required by the team

To ensure standards are in place and are adhered to by all.

Support the implementation of effective systems and processes for infection control management and relevant audit

4. Knowledge, Training and Experience

Maintain own clinical development by keeping abreast of new treatments and technologies

To develop and maintain a positive learning environment by providing support, ensuring development requirements are addressed.

To assist in the provision of clinical supervision and provide the opportunity to reflect and discuss areas of practice.

• Registered Nurse - Child (if applying for the paediatric post) (RSCN/RNC) with current NMC registration
• Relevant Degree or Equivalent
• MSc in Advanced Practice or Relevant ANP courses at Level 7, or equivalent in, Advanced Physical Assessment skills and paediatrics
• Desirable
• A Non-medical prescribing qualification

• Ability to manage and prioritise workload
• Proven ability to work as part of a team
• Demonstrate enthusiasm and self-motivation
• Understanding of autonomous practice
