Job summary

An exciting opportunity has arisen for an experienced and self-motivated GP Practice Nurse to join our friendly, multi-disciplinary nursing team, providing high quality patient focused care

Main duties of the job

To deliver evidence based care for patients with long term conditions, as well as preventative nursing interventions. Key areas of focus would be

- chronic disease management (desirable but not essential as training and support would be provided)

- routine immunisations

- wound care

- women's health, including contraception management, cervical screening

- other core general practice nursing skills

- opportunities to develop new practices to improve patient outcomes, supported by our experienced and dedicated clinical, leadership and administrative team

- demonstrates an understanding and commitment to safeguarding practices and be willing to undertake safeguarding training

- willing to share learning and support other team members in their development

About us

We are proud to be part of Wyvern Health Primary Care Network, a group of 3 GP practices, along with Old Town Surgery and Priory Road Medical Centre.

Our multidisciplinary team includes, GPs, Practice Nurses, Clinical Pharmacists, Pharmacy Technicians, Physiotherapist and Phlebotomist, supported by our excellent and experienced administrative teams.

We are a high achieving practice for National GP Patient Survey and Friends and Family results, demonstrating an excellent quality of care and understanding of our patient population needs.

Job description

Job responsibilities

The post holder is a registered nurse who acting within their own professional boundaries will provide general practice nursing to the whole of the Practice population.

As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision making in the management of patients. They will work collaboratively within the general practice team to meet the needs of patients and support the delivery of policy and procedures.

Key Responsibilities

Clinical Practice

o Assess, plan, develop, implement and evaluate programmes to promote health and well-being.

o Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition as appropriate

o Work with other health care professionals to diagnose, monitor and treat long term conditions, including non-drug based treatment methods using a management plan and in line with national and local policies and practice needs

o Work with patients in order to support adherence to prescribed treatments.

o Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency

o Implement and participate in vaccination and immunisation programmes for both adults and children

o Promote and deliver evidence-based care to patients

o Meet the needs of patients presenting for opportunistic wound care

Communication

o Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

o Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural backgrounds and preferred ways of communicating.

o Anticipate barriers to communication and take action to remove or reduce those barriers

o Act as an advocate when representing the patients and colleagues viewpoints to others

Delivering a Quality Service

o Recognise and work within own competence and professional scope of conduct as regulated by the Nursing and Midwifery Council (NMC)

o Produce accurate, contemporaneous and complete records of patient consultations, consistent with legislation, policies and procedures

o Prioritise, organise and manage own workload in a manner that maintains and promotes quality

o Deliver care according to the NSF and National Institute for Clinical Excellence (NICE) guidelines and evidence based care

o Assess effectiveness of care delivery through self and peer review, bench marking and formal evaluation

o Participate in the maintenance of quality governance systems and processes across the organisation and its activities

o In partnership with other clinical teams, collaborate on improving the quality of healthcare, responding to local and national policies and initiatives as appropriate

o Evaluate the patients response to health care provision and the effectiveness of care

o Support and participate in shared learning across the practice and wider organisation

o Participate in the management, review and identify learning from patient complaints, clinical incidents and near-miss events utilising a structured framework.
