Practice Nurse - Primary Care

Job summary

We have a forthcoming permanent vacancy for a Practice Nurse for 36 Hours per week. There is a further 9-month temporary contract for a Practice Nurse, covering maternity leave, for 16 Hours per week. The role will be responsible for the delivery of practice nursing services to our patient population, working as part of the practice multidisciplinary team, delivering care within a scope of practice. The nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management and health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

We have a low turnover of colleagues, which we like to think reflects how we choose to work as a team.

The successful applicant will be required to undertake the full range of nursing, with most reasonable patient slot numbers per session.

Main duties of the job

Treatment Room Sessions

  • To see & manage patients in booked treatment room sessions in your own clinic
  • Patients will be seen for:

Administration of injections

Childhood & travel immunizations

Ear syringing

Dressings and minor injuries

The practice nurse will also:

Assist medical colleagues in the carrying out of minor surgical procedures and the fitting of contraceptive devices

Provide telephone advice for patients and refer as appropriate to other members of the team

Take part in other appropriate patient care as requested (for example the need to chaperone medical colleagues during intimate examinations)

Phlebotomy is usually carried out by the HCA/phlebotomist but, depending on demand or sickness, there will be a need to see patients for venepuncture

Chronic Disease Management and Health Promotion sessions

To undertake chronic disease management and health promotion. This will depend on the skills of the post-holder.

To include:

General assessment and lifestyle advice e.g. for patients with obesity.

Assessment and ongoing checks for Hypertensive patients

Annual reviews for patients with cardiovascular disease

Annual reviews for patient with diabetes

Annual review for patients with asthma and COPD

  • Cervical cytology
  • Minor Surgery Procedures
  • Wound management
  • Other appropriate work linked to the Quality and Outcome Framework

Our Practice is just off the City centre with our own Car Park. We have a patient size of 10,700 and commit to, todays work today, with systems and care; that experiences high patient satisfaction, whilst protecting colleagues. Great value is placed on work life balance. We offer good working conditions in a supportive and friendly environment; with a manageable workload and great team.

We are a Training Practice with teaching and coaching at all levels, from Students to GP Trainee Registrars. York Street is also Research active.

There is adaily coffee meeting plus a timetable of regular educational meetings.

We operate with ten GP's, 3 Practice Nurses, 2 Healthcare Assistants, with wider services such as a Clinical Pharmacist, Social Prescriber, Physios, Specialist Mental Health & Midwifery on site. There is a multiskilled administration team who all cover each others roles.

The Practice has its own Staff Car Park.
