There are Full-Time & Part Time
Clinical Pharmacy openings within the City Primary Care Network (PCN)
practices. Primarily based at Arbury Road Surgery, York Street Medical Practice
& Bottisham, duties will also be undertaken at other Practices as outlined
below:

The whole PCN welcomes & supports
flexible working patterns and job share, applications. The PCN has 1.8 Full
Time Equivalent hours vacant.
The PCN is South of the CIty and a compact & good performing Care Network
Main duties of the job
Responsible for implementing an effective
medicine management, including cost-effective prescribing, working as an
independent prescriber or willing to work towards independent prescribing
status are main aims of the role. The post-holder will also be part of the
multi-disciplinary team, in line with the strategic objectives of the PCN.
The successful
applicants will be required to undertake the full range of clinical pharmacy
work, with most reasonable patient slot numbers per session. We support the
development of special interests including for example care home residents,
frailty, dependence forming medications, cardiovascular disease.

Training Practices with teaching and coaching at all levels, from Students to
GP Trainee Registrars.There
are timetables of regular educational meetings.
Practices are SystemOne
Clinical IT, Ardens & Accurx. Multiple choices are available for patient
appointment consultations and we use electronic prescribing.

The post
holder will work within their clinical competencies as part of a
multi-disciplinary team. The role will provide expertise in clinical medicines
management, provide face-to-face structured medication reviews, manage
long-term conditions, management of medicines on transfer of care and systems
for safer prescribing, and manage repeat prescription authorisations and
reauthorisation, and acute prescription requests.
The post
holder will perform face-to-face medication review of patients with
polypharmacy, especially for older people, people in residential care homes and
those with multiple co-morbidities; also minor ailments. The role will provide
leadership on quality improvement, clinical audit and manage some criteria in
the Quality and Outcomes Framework.
This
role is pivotal to improving the quality of care and operational efficiencies to
deliver an excellent service within general practice.
The post
holder will receive support to develop their role to become an independent
prescriber, if they do not hold that qualification.

- At least 3 years post registration relevant experience
